The Harbor Court Deploys Disease Care Management Program Daylight IQ™ to Enhance Resident Quality of Care

Rocky River, OH -- The Harbor Court Independent and Assisted Living Community has implemented the COMS Interactive, LLC Daylight IQ™ disease management program as a core service offering to its residents. Daylight IQ™ is delivered through an integrated software system of standardized processes and disease care management, which has a proven record of improving overall quality of care and dramatically reducing hospital admission rates.
Nursing care serves as the backbone of an Assisted Living facility. The Harbor Court offers nursing services to both its Independent and Assisted Living residents. With the Daylight IQ™ disease management system, The Harbor Court residents receive the additional benefit of clinical point-of-care technology that guides the nursing team based upon a customized health profile for each resident, to immediately identify any minor health changes. Through early detection of small negative changes in condition, the nursing team is able to respond quickly, preventing further progression of the illness or affliction.
